Background
Accurate recording of fracture classification and planned surgical management in trauma MDT meetings underpins safe handover, supports reporting to the National Hip Fracture Database and aligns with NICE CG124 recommendations for hip fracture management. A recent review at Guy’s & St Thomas’ revealed wide variation in documentation of fracture type and planned procedure in MDT notes, identified as a problem by clinical teams .
Aims
To improve completeness of documenting fracture classification and planned surgery in trauma MDT meetings at Guy’s & St Thomas’ during the audit cycle.
Objectives
Audit scope: Review all adult neck of femur cases presented in trauma MDT meetings from 1 December 2023 to 31 March 2024 to assess whether fracture type and planned surgery are documented.
Template prompt: Implement a structured prompt in the electronic MDT proforma by 31 January 2024 to capture both fracture classification and intended management.
Training: Deliver a brief training session for orthopaedic juniors and registrars on using the updated MDT template by 15 February 2024.
Midpoint review: Conduct a midpoint audit by 15 February 2024 to identify any barriers to complete documentation and refine the process as needed.
Re-audit: Re-audit MDT documentation by 31 March 2024 to measure improvement in completeness against the 100% documentation target.
