Why this project was needed
Hip fractures affect approximately 76,000 people per year in the UK and carry significant mortality. NICE CG124 sets out evidence-based standards for their management including timing of surgery, orthogeriatric assessment, and post-operative documentation requirements. A review of operated hip fracture records at GSTT identified inconsistencies in operative note completion, specifically regarding the data elements required for accurate NHFD submission and compliance with NICE guidance on surgical documentation.

To improve compliance with NICE CG124 guidance in the documentation of operated hip fracture records at GSTT, ensuring operative notes contain all data elements needed for accurate NHFD entry.
