Improving Trauma Meeting Documentation for Neck of Femur Fractures

Why this project was needed
Accurate documentation of fracture classification and planned surgical management in the multidisciplinary trauma meeting is essential for NHFD data collection, accurate coding, and the safe planning of operative care for neck of femur fractures. A review of MDM trauma meeting notes at GSTT identified inconsistent recording of NOF fracture sub-type (intracapsular vs extracapsular, Garden classification) and planned surgical approach, which was adversely affecting data quality and NHFD submission accuracy.

To establish the current rate of complete documentation of NOF fracture classification and planned surgery in MDM trauma meeting notes, and to improve this through a targeted intervention.

Specific measurable steps
- Retrospectively review MDM trauma meeting notes for adult NOF fracture admissions between December 2023 and March 2024.
- Determine the proportion of cases where fracture type and planned surgery are documented.
- Implement a structured smart text template for trauma meeting documentation.
- Re-audit following implementation to assess improvement in documentation completeness.
