Background

Why this project was needed

Neck of femur fractures represent one of the highest-volume trauma admissions in the NHS and are associated with significant morbidity and mortality in an elderly population. Accurate and complete admission proforma documentation is essential for NHFD data submission, Best Practice Tariff compliance, and safe handover between clinical teams. An audit of proforma completion rates was undertaken following concerns about inconsistent documentation quality across weekend and on-call admissions.

Aims

What the project set out to achieve

To assess compliance with NICE guideline requirements and the Best Practice Tariff criteria in completion of the NOF fracture admission proforma at GSTT.

Objectives

Specific measurable steps

  1. Retrospectively review NOF proformas for all neck of femur fracture admissions over the audit period.
  2. Assess documentation of time from admission to surgery, AMTS score, FiB nerve block, and orthogeriatric review.
  3. Identify where proforma fields are most frequently incomplete or omitted.
  4. Implement smart text tools or clerk-in checklists to improve documentation quality.
